This 33-year-old patient – smoker of half a packet a day – came to us with no relevant medical history, having had a cyst in the region of the right mandibular body in 2007. He had had extractions of the affected teeth – 43, 44, 45 and 46 – and was operated on that same year by colleagues who performed cystectomies and apicoectomies on the teeth involved.
The following year, the roots were exposed and the patient was still weeping at the site of the wound. The patient continued in this state of accompanying bone destruction and was operated on again in 2011 without successful halting of its advance.
This year, he came to us and we extracted teeth 44, 45 and 46, cleaned the entire cavity and waited 6 weeks for the soft tissues to close. They closed, stopped weeping, and the cystic pathology stopped.
After 6 weeks, we proceeded to reconstruct the defect with an autograft from the ipsilateral retromolar area, through a classic approach.

Hola Juan. Buen trabajo el vuestro. Me gustaría saber como aportáis vascularización al injerto y que usáis de relleno del espacio entre el injerto y la cresta alveolar. Usáis después alguna membrana de colágeno, bovino, porcino, etc Un saludo

Muchas gracias. Normalmente en este tipo de reconstrucciones hacemos un abordaje en tunelización para mantener la integridad del periostio y así su potencial vascular.
En este caso lo hicimos abierto porque queríamos asegurarnos de que había quedado limpio el lecho tras tantas cirugías previas que llevaba el paciente en la zona.
Para el relleno usamos 100% material autólogo de la zona retromolar, ya que es de este modo como obtenemos los mejores resultados con las biopsias y consideramos el hueso autólogo como el “Gold Standard” .
No colocamos membranas ya que estas tienen una doble finalidad, una sujetar el hueso particulado y dos evitar que el tejido blando avance dentro de la cavidad pero en este caso el bloque de hueso está fijado con tornillos de osteosíntesis que lo mantienen inmóvil y contienen al hueso particulado, y no va a avanzar dentro de la cavidad porque hay un bloque de hueso en medio, por crestal también se lo impide el hueso particulado compactado.
Además poniendo una membrana entre el periostio y nuestro injerto limitaríamos la capacidad osteogénica de este.
